We ask that you complete the following form before each BOD meeting.
Some of our BOD members are not heading up a committee, so there is no need to complete the form unless you need to add an item to the agenda (see the drop-down on committees and select Add Agenda Item).
- Please complete the form by Wednesday (2 PM) before the monthly BOD meeting. If you don’t get the chance to submit your report before the deadline we ask that you bring enough printed copies with you on the day of the BOD meeting.
- Our administrator will gather all the forms and turn them into a PDF and distribute them to the BODs by Friday or Monday prior to the meeting.
- Below find a section to be completed it you need to add something to the agenda.
